(Bile) Flow State

Bile. It’s something most people don’t think about. The liver continuously makes it. The gallbladder stores and concentrates it. And when food arrives, particularly fat, the gallbladder contracts and sends bile into the small intestine, where it gets to work. This whole pathway: liver → gallbladder → small intestine… that release is bile flow.

Most of us only encounter bile when something goes wrong. But this yellow-green digestive fluid is doing important work every day. It helps us digest and absorb fats and the fat-soluble vitamins A, D, E, and K. Bile acids also act as signaling molecules involved in metabolism and help shape the microbial environment of the gut. And bile provides a route for the body to eliminate cholesterol, bilirubin, and some metabolized hormones.

So bile flow is less niche than it sounds. And something Jelena thinks about more than most.


READY. SET. FLOW!

Bile begins in the liver, where bile acids are made from cholesterol. From there, bile travels through the biliary system. Between meals, much of it is diverted into the gallbladder, where it is stored and concentrated until food arrives.

When we eat, especially a meal containing fat, digestive signals tell the gallbladder to contract. Bile travels through the common bile duct and empties into the first part of the small intestine.

Again: Liver → gallbladder → small intestine.

Once there, bile acts a little like dish soap on grease. Fat and water don’t naturally mix, so bile emulsifies dietary fat into smaller droplets, giving digestive enzymes much more surface area to work with. This makes fats, essential fatty acids, and fat-soluble vitamins easier for the body to absorb.

But digestion is only part of the story. The liver also packages substances for elimination through bile. Cholesterol and bilirubin leave the body this way, and the liver also uses bile as one route for clearing estrogen from the body. Some of what enters the intestine through bile can subsequently be reabsorbed, which is one reason the liver, bile, gut, and microbiome are better understood as a connected system rather than separate departments.


BILE HAS A BODY CLOCK

Bile isn’t operating independently of the rest of us. Bile-acid synthesis and metabolism follow daily rhythms, and feeding and fasting cycles influence the size and composition of the bile-acid pool.

That puts some very unglamorous habits back on the table: regular meals, adequate hydration, daily movement, daylight, and reasonably consistent sleep. None is a magic bile-flow hack. Together, however, they support the circadian and digestive systems in which bile is produced, released, recycled, and eliminated.

This is also where slowing down matters. Digestion is coordinated through hormonal and neural signals, including communication along the gut-brain axis. Eating in a chronically rushed or distracted state isn't the same thing as having a bile disorder, but giving the body time to register, anticipate, and digest a meal is a pretty reasonable place to start.

Water matters, too. Jelena often adds mineral salt and lemon to water as an easy hydration ritual. The salt isn't a bile stimulant; its there as an electrolyte. The lemon brings acidity, flavor, and, perhaps most importantly, makes the ritual something you’re likely to actually drink.


GET BITTER

Bitterness gets the digestive system’s attention.

We now know that bitter receptors aren't confined to the tongue. They're also found throughout the gastrointestinal tract, including on cells involved in the release of digestive hormones. When bitter compounds activate these receptors, they can influence gastric secretion, gut motility, appetite signaling, and other processes involved in preparing for and responding to food.

The research on individual bitters in humans is still developing, so “eat something bitter and your gallbladder will empty” is far too simple. But there is real physiology behind the longstanding relationship between bitter plants and digestion.

Fortunately, bitter isn’t difficult to find.

  • Radicchio
  • Endive
  • Dandelion greens
  • Mustard greens
  • Chicory
  • Artichokes
  • Broccoli rabe
  • Grapefruit*
  • Lemon, lime, and orange peel

Herbal bitters provide a more concentrated version. They typically combine bitter roots, barks, peels, seeds, and herbs and are traditionally taken shortly before or around meals.

Fresh ginger can be steeped in hot water or incorporated into meals. ACV can be diluted in water around mealtime if it's well tolerated. Nettle makes an easy mineral-rich herbal infusion.

WHAT HAPPENS WHEN BILE DOESN'T FLOW WELL?

True impairment of bile formation or flow is called cholestasis, and that's a medical condition, not something to self-treat with bitters or a castor oil pack. Gallstones and other problems can also obstruct bile ducts and require medical evaluation.

But bile and digestive health exist on a continuum, and symptoms involving fat digestion can sometimes be clues that warrant a closer look.

Depending on the cause, impaired bile delivery can interfere with the digestion and absorption of fats. Over time, significant impairment can also compromise absorption of vitamins A, D, E, and K.

Symptoms such as persistent upper-right abdominal pain, jaundice, very pale or clay-colored stools, dark urine, fever, or severe pain after eating deserve medical attention rather than a wellness protocol.

For less dramatic but persistent digestive complaints, the investigation can be more complicated.


BILE + THE MICROBIOME

Jelena sees plenty of people with bloating, gas, abdominal discomfort, irregular digestion, and other gut complaints without an immediately obvious explanation.

One possible culprit is SIBO: small intestinal bacterial overgrowth.

The large intestine is supposed to contain an enormous microbial population. The small intestine is a different neighborhood. It contains bacteria, but ordinarily at much lower densities. SIBO occurs when excessive numbers of bacteria colonize the small intestine, where they can ferment food before it has been fully digested and absorbed.

That can mean bloating, gas, abdominal discomfort, diarrhea or constipation, and, in more significant cases, nutrient malabsorption.

Bile is one of several mechanisms that help keep the microbial population of the small intestine in check. Bile acids can directly inhibit some microbes and also participate in signaling that supports the intestine's antimicrobial defenses. Intestinal motility, stomach acid, immune defenses, and anatomy matter too.

This relationship also goes both ways: bile shapes the microbiome, and the microbiome transforms bile acids. Gut bacteria convert primary bile acids made by the liver into secondary bile acids, while changes in the bile-acid pool can alter which microbes thrive.

Research supports an association between disrupted bile dynamics and bacterial overgrowth, but SIBO shouldn't be reduced to “poor bile flow.” There are numerous possible causes and risk factors, including impaired intestinal motility and structural or anatomical problems.

That’s why persistent symptoms are worth investigating rather than endlessly eliminating foods.


KEEP IT MOVING

Supporting digestion doesn't necessarily require turning every meal into a supplement regimen.

Eat regularly. Include healthy fats rather than avoiding them entirely. Bring bitter greens and herbs into the rotation. Hydrate. Move your body. Pay attention to sleep and light. Let meal time linger. And when in a pinch… Castor oil packs have been used traditionally as an external practice for abdominal and digestive support. The setup is wonderfully analog: castor oil, a piece of flannel, warmth, and nowhere else to be for a little while.

Traditionally, flannel is saturated with castor oil and placed over the upper-right abdomen, roughly over the liver and gallbladder area. A barrier and gentle heat are placed over it while you rest.

So why does Jelena prescribe them?

Because not every supportive practice needs to be “science”. Warmth, rest, abdominal relaxation, and the ritual itself can have a place in a larger digestive routine. Castor oil packs have a long history of traditional use, and when used appropriately, they’re an easy external practice to pair with all the internal work happening at a meal.


THE BIGGER PICTURE

Bile is a good reminder that digestion is never just about the stomach.

TL;DR Eat the radicchio. Use the bitters. Drink some water. Go outside. Move around. Sleep. Give digestion a chance to anticipate a meal instead of constantly surprising it. And every once in a while, lie down with a warm castor oil pack and do absolutely nothing.

It begins with the liver manufacturing bile from cholesterol. It involves the gallbladder storing and releasing it at the right moment. It depends on the small intestine, digestive hormones, nerves, microbes, feeding rhythms, and the recycling of bile acids back to the liver so they can be used again.

Which makes supporting bile flow less about finding one heroic supplement and more about supporting the system bile belongs to.
